Advance Directive Form

Indiana’s Advance Directive form can be found at the bottom of this page in English or Spanish. It specifies an individual’s living and last will, instructions regarding their health care and treatment in different conditions, and healthcare proxy. It must be signed by the individual, proxy, and witnesses. Full information can be found in the official brochure.

How to Order a Death Certificate

Only people presenting direct and proven interest can obtain a death certificate in Indiana, such as immediate family members and their legal representatives.

The required application information includes:

  • Deceased’s name and date of birth
  • Names of deceased’s parents and maiden name of mother
  • Place and date of death
  • Applicant’s name and proof of relation to the deceased
  • Applicant’s phone number and address
  • Copy of applicant’s government-issued photo ID
  • Preferred delivery method
  • Number of copies required

The certificate and one certified copy cost $8. An additional copy ordered at the same time costs $4.

Indiana’s SB 253 Revised Uniform Fiduciary Access to Digital Assets Act authorizes an individual’s legal representative who can access and control their digital assets and communications. This bill came into effect on March 23, 2016.
