Kansas’s Advance Directive form specifies an individual’s living and last will along with detailed instructions on their health treatment and authorizes a healthcare proxy. The form must be signed by the individual, proxy, and a witness.

How to Order a Death Certificate

Death certificates in Kansas are only issued to immediate family members, their legal representatives, or people presenting proven direct interest in the deceased’s property.

The following information is required to obtain a death certificate in Kansas:

  • Deceased’s name, race, and sex
  • Deceased’s place and date of birth
  • Deceased’s marital status and name of spouse
  • Date and place of death
  • Applicant’s name, phone number, and mailing address
  • Applicant’s relation to the deceased and proof of the fact
  • Copy of applicant’s government-issued photo ID

The certificate with one certified copy costs $15. An additional copy costs $15 with an $11 card processing fee if the request is made over the phone. Cash is only accepted when the request is made in person.
